We specialise in caring for clients with complex, and chronic health care needs.  We work closely with Medical Specialists, GPs and other members of the multidisciplinary team to implement and facilitate sustainable health management plans.

We are able to provide nursing care plans, and can provide a Clinical Nurse Consultant (CNC), experienced in Delegation of Care models to provide and implement training and education.

 

We are also able to provide a Diabetes Clinical Nurse Consultant (CNC) with experience in Type 1, Type 2, MODY, LADA, insulin pumps, glucose flash monitors, and continuous glucose monitors (CGM). We will liaise with your Endocrinologist, GP and allied health team to ensure the best management possible.
